Imperial Health Adult Day Health Care Center is an organized day program providing medical, social, and restorative health services in a health care center, serving the frail elderly and other adults with physical or mental impairments.

ADHC Services
Adult Day Health Care, designed to serve medically, physically and/or cognitively impaired adults, provides a structured day program which includes nursing, social services, restorative therapies such as physical, occupational, and speech therapies, dietition and mental health therapist.
Meals, and transportation are also provided in a warm, caring, and professional environment. This program focuses on person centered care for each person based on the participant's plan of care.

MEDICAL SOCIAL WORK
• Supportive counseling with community resource referrals
PROFESSIONAL REHABILITATION SERVICES:
- Registered Dietitian
- Psychiatric/Psychological Services
- Physical Theropy
- Occupotional Therapy
- Speech Therapy
SKILLED NURSING SERVICES
- Registered Nurses on-site daily to evaluate and provide medical and nursing care coordination.
THERAPEUTIC ACTIVITIES
- Designed to stimulate, educate, and increase socialization with participants.
TRANSPORTATION SERVICES
BREAKFAST & LUNCH
Eligibility
Individuals may be eligible if they meet all of the following criteria:
- Have a medical condition that requires treatment or rehabilitative services prescribed by a physician.
- A Physical and/or mental impairment that limits activities of daily living, but not so serious as to require 24-hour institutional care.
- A reasonable expectation that preventative services will maintain or improve the present level of functioning.
- A high potential for further deterioration and probable institutionalization if ADHC services were not available.
Program Costs
For CBAS eligible participants, program services may be covered by the following insurances:

- LONG-TERM CARE INSURANCE
- PRIVATE PAY
